    The nursing care plan is a five-step framework, which is also known as ADPIE (Assessment, Diagnosis, Planning, Implementation, Evaluation):

    ● Assessment (data collection) requires you to gather subjective data, such as symptoms, and objective data, such as lab reports.

    ● For the nursing diagnosis, you have to give the clinical judgement of actual or potential health problems.

    ● For planning/outcomes, you must set goals using the SMART framework.

    ● Additionally, the implementation of interventions allows you to take evidence-based actions to ensure better patient outcomes.
